A CASE OF CYSTIC ACNE UNRESPONSIVE TO MEDICAL TREATMENT

I see many patients for various types of acne. My treatments vary from patient to patient and within each patient the treatments are modified according to response. Generally our Acne treatments include lifestyle modification, topical skin care, Acne cyst extractions, medical microdermabrasion, Medicated Acne peels, Intense Pulse light, PDT and short course of oral antibiotics if indicated.

More than 80% of patients have great response. I see 80-90% improvement in 3-4 months and we switch to maintenance phase of acne management.

Most acne treatments are done as Medical treatments and billed to medical Insurance (except PDT).

Recently I experienced something rather unusual. One of my teen patients with moderately severe cystic type acne with moderate inflammation who has been under my care for over 6 months has had a frustrating course. He would get better for few days, then get worse.He has been on topical and oral antibiotics longer than any other patient in my practice. Antibiotics transiently improved his lesions but stopped working after a few days. He has had microdermabrasion, chemical peels, Intense pulse light regularly.

His cystic lesions and inflamed skin improved most notably with IPL treatments which led to less redness, and fewer new cysts but this effect did not last beyond a few days

About 3 weeks ago, after detail discussion with his mom, we decided to stop all oral antibiotics, institute a stricter diet (less sugar, less saturated fat, no dairy) and try an atifungal medication. He had history of fungal skin infection in the past, some areas on face clinically appeared to be consistent with fungal infection.

He has shown remarkable improvement since then. I plan to continue IPL treatment every 2 weeks to reduce redness and cyst formation. I saw him today he looked GREAT!
He has NO active cystic lesions, has mild to moderate redness and acne scars which we can reduce over the next few months with microdermabrasion or a topical.

It just makes me realize that sometimes you have to think out of the box to get results!

I am really appreciative of my patient and his mother. If it was not for their patience and understanding, we would not have achieved this!!

Accutane No Longer Sold

Generic Versions Still Available as Isotretinoin

July 8, 2009 - Accutane, Medication for cystic acne, will not be sold any more, drug giant Hoffmann-La Roche Inc. has announced.

Isotretinoin, The Generic version of Accutane is still available through various manufacturers. Roche introduced Accutane in 1982 and has sold it to 13 million patients since its introduction will not be selling Accutane any longer.

In a news release, Roche announced its decision was due to "business reasons". Accutane sales have been declining over the years due to availability of generic Isotretinoin, now reduced to less than 5% of the Isotretinoin market.

Personal injury lawsuits make up another big reason. Inflammatory bowel disease was recently added to long list of serious side effects possibly caused by this medication.
Accutane and other Isotretinoin products are effective treatments for serious Cystic acne. But this class of drugs has been associated with many serious side effects: Depression, psychosis, suicide, Inflammatory Bowel disease are just a few of the long line of side effects. Accutane and Isotretinoin products can also cause miscarriage and serious birth defects if taken by pregnant women.

I have been effectively treating all types of Acne with a combination of lifestyle adjustment, Topical skin care, prescription antibiotics and office treatments. Cystic Acne is generally resistant to treatment and can be associated with deep scarring. In our office we have been able to achieve reasonable control with Photodynamic therapy which also helps reduce scarring.

Acne Vulgaris - New Treatments

Acne treatment has come a long way in the last decade! Medical research has led to new discoveries in causes of acne and developed many new acne treatment options.Combination treatments are the most effective method for long term control. Each acne treatment works on one cause of acne. Combining two or more treatments leads to attacking at several causative levels ensuring better control.

Acne is a chronic condition and requires a multilevel approach for effective management. When treating Acne, We can achieve a reasonable control in more than 90% of our patients keeping in mind the following:

  • It is a matter of time and patience. While most of our patients achieve clear skin in 3 to 4 months, for some it may take longer.
  • Same treatment protocol does not work for everyone.
  • Lifestyle, Diet, exercise, stress management play a very important role in effective Acne management.
In our practice we divide Acne treatments into these categories:
  • Topical skin care. For most of our patients we recommend our own skin products with Alpha and beta hydroxy acids, Salacylic acid, Sulphur and anti inflammatory botanicals. These products are less irritating than prescription Retinoids
  • Oral Antibiotics: I use oral antibiotics ONLY when clearly indicated for inflammatory Acne and for the shortest time possible. Long term use of oral antibiotics has been associated with development of bacterial resistance, yeast infections and other side effects.
  • Acne Surgery for gentle extraction of comadones combined with Medical Microdermabrasion and Medicated Chemical Peels reduces oil production, blackheads and inflammation leading to fewer acne lesions.
  • Light based treatments especially Photodynamic therapy work effectively for cystic acne resistant to other treatment modalities.
  • Resurfacing lasers such as CO2 Fractional laser, work very well for reduction of Acne scars

Stress Exacerbates Teen Acne

Medical studies show, stress worsens Acne in teens. Teenagers under high levels of stress are more likely to have increased acne severity.

Researchers say acne is an inflammatory disease, and previous studies have shown stress can trigger inflammation in the body. This is most likely mediated through release of Adrenal Androgens during stress which causes increase in sebum production and inflammation.

Knowing what causes stress and developing an effective stress management strategy is an important adjunct to long term acne management. I have found, discussing the role of stress in Acne exacerbation and the importance of adequate stress management leads to more realistic expectations from medical treatments and fewer inflammatory Acne breakouts.
