BIOIDENTICAL HORMONE REPLACEMENT THERAPY overview in Southlake Texas


Until recent years, Hormone Replacement Therapy generally consisted of synthetic hormones given to menopausal women for symptoms of hot flashes, vaginal dryness etc.

Premarin was the first brand of hormone for HRT in USA and contained only equine (horse) estrogens. Due to concerns over increased incidence in uterine cancer with use of Premarin, in the 1950's Prempro was introduced which contains a synthetic progestin in addition to Estrogen.

In 2002, the WHI (Women's health Initiative) published its results looking at the effects of HRT with Premarin and Prempro on a large group of women. The findings were shocking and created a frenzy in patients taking the hormones and doctors prescribing them. HRT with synthetic hormones was found to definitively increase the risk of blood clots, stroke and breast cancer.

Millions of women stopped their HRT following the release of these results and started to look elsewhere for the relief of their symptoms.

Bio-identical Hormone therapy BHRT

Bio-identical means that the biochemical structure of the hormone is identical to the hormone produced by a woman's body. Not a horse's hormone or an artificial formula close to but not quite the same as a woman's hormone.

What took so long for BHRT to gain acceptance?

The pharmaceutical companies developed synthetic estrogens and progestins to patent their drugs. They assumed small changes in estrogen or progesterone molecule will not affect women, which unfortunately is not true.
Birth control pills are synthetic estrogens and progestins,and most women know each brand has different effects.Some of the benefits from BHRT are:

Anti Aging Hormone Replacement


As a Family Practitioner, I have treated patients with estrogen, progesterone and testosterone for many years. I could not understand why patients would have a less than optimum response in spite of adequate hormone dose.

Comprehensive Bio-identical Hormone Replacement Therapy (BHRT) involves optimization of all hormone systems because very often their functions are interdependent. Optimizing one area without addressing the others can lead to frustration.
The hormones I assess in my initial workups on all patients and consider replacing on an individual basis include:


  • Testosterone

  • Estrogen

  • Progesterone

  • Pregnenolone

  • Human Growth Hormone

  • Thyroid Hormone

  • DHEA

Parallel to any good hormone replacement regimen is the program of lifestyle optimization that will allow our bodies to maximize the rejuvenating effects of BHRT on our body cells.

One of the major concepts I learned during my certification with anti aging medicine is: "We age because our hormones decline, our hormones do not decline because we age" which is contrary to what I learned and practiced for many years in mainstream medicine.

The WHI study published in 2002 was shocking for many patients on traditional HRT and for the doctors prescribing HRT Some of the hesitation may be related to abuse of Growth hormone and testosterone by body builders and athletes and all the side effects resulting from abuse.

Conventional medicine is slowly starting to recognize the reasoning of BHRT as more and more evidence accumulates about the safety and quality of life benefits from BHRT

Hormone Replacement therapy is not for everyone. If you have history of malignancy of sexual organs (ovaries, uterus, breasts in women), (Testes, prostate and breasts in men) you may not be a candidate for HRT.

Dallas Anti Aging Update: Exercise and Aging

In a recent study German Scientists compared cell life spans of different groups of subjects. The first group consisted of professional runners in their 20s, The second group consisted of sedentary young individuals, the third of middle aged sedentary people and finally a group of middle-aged longtime runners ( average age of 51) running on average 50 miles per week.

Dr. Christian Werner, an internal-medicine resident at Saarland University Clinic in Homburg noted the striking difference in appearance. The active middle aged group looked remarkably younger than the inactive counterpart.
The findings at the cellular level were even more striking. While the white blood cells from each of their subjects in both the active and sedentary young adults had similar-size Telomeres, the Telomeres in the sedentary middle aged group were significantly shorter than the active counterpart.

Telomeres are small end caps on the DNA strands. With each cell division, part of the telomere cap is snipped to repair the rest of the DNA strand. With repeated cell replication telomeres get shorter and shorter until the cell dies or is unable to repair or replicate itself. Telomere length is widely accepted as a reliable marker of cell age.

Exercise has many benefits, increased Aerobic capacity, better Muscle tone, slimmer Physique, but the effect of regular exercise on cellular aging could be astounding. Thomas LaRocca is a Ph.D. candidate in the department of integrative physiology at the University of Colorado in Boulder, who has just completed a new study on the effects of exercise on aging cells. In Mr. LaRocca's study, people were tested both for their V02max (Maximum aerobic capacity) and their white blood cells' telomere length. In subjects 55 to 72, a higher V02max correlated closely with longer telomeres. The fitter a person was in middle age or onward, the younger their cells.

How does activity affect the DNA? we do not know the answer yet, however Dr. Werner found that his older runners had more activity in their telomerase, a cellular enzyme postulated to help protect and lengthen telomeres. Exercise may have an effect on activity of telomerase and not directly telomeres .

No one knows how much exercise is needed to lengthen cellular lifespan. Any amount of regular physical activity helps keep you healthy, strong and young.
