5 Skin Conditions That Respond Well To IPL Photorejuvenation

IPL Photo-rejuvenation is a treatment done with an Intense Pulsed Light source. This type of treatment is effective for treating skin conditions in the superficial layers. It is associated with some discomfort, most patients describe the treatment as a feeling of "rubber band snap" against the skin. Intense Pulsed light is absorbed by brown and red pigment, making the treatment more uncomfortable in people with dark pigmentation and redness.
IPL is quite effective and well tolerated in the appropriately selected patient group.
IPL is usually done without topical anesthesia. In our office , we use very cold gel and ice packs to reduce discomfort. I like to discuss the advantages, realistic expectations and the limitations of this treatment before starting. IPL treatment is usually done as a series of 3-5 sessions, 2-4 weeks apart. I frequently combine this treatment with procedures like Microdermabrasion, Dermawave facial toning for synergistic effect.
I have found with proper patient selection, IPL can help the following skin conditions:

  • Superficial Brown Spots/sun Spots

  • Facial Redness/Small broken Blood vessels

  • Rosacea

  • Cystic Acne lesions, unresponsive to conventional Treatment

  • Superficial Photoaging manifested as rough texture, brown discoloration, fine lines etc.

If you are considering Intense Pulse Light Treatment also known as Photo rejuvenation or Fotofacial, please call our office at 888-210-9693 for more information or to make an appointment.

Number 1 Breakthrough for Anti Aging Skin Treatment: Dallas Fort Worth Skin Care Update

A recent article on Web MD called it "The number 1 Anti-aging breakthrough"

Fractional CO2 Laser Resurfacing
Traditional carbon dioxide laser resurfacing has been the gold standard for facial wrinkle reduction for many years. However, the treatment is done under general anesthesia and can potentially have more than a few undesirable side effects. The newer, safer Fractional CO2 resurfacing consistently delivers Potent results without the detrimental complications of traditional CO2 laser.

I decided to add this treatment to my practice over a year ago and tried many types of CO2 Fractional laser devices before making my decision. I chose the device after considering several factors, patient comfort, recovery after treatment, flexibility of treatment parameters and of course clinical results.
I use DOT Fractional CO2 for skin resurfacing

The laser removes the superficial layers of skin improving texture, fine lines and superficial pigment irregularities AND penetrates deep into the dermis stimulating collagen production leading to reduction of deeper wrinkles and scars.

Advantages of Fractional C02 Laser over traditional CO2 laser

  • Far less incidence of post Laser scarring
  • Less Incidence of Pigment disorders
  • Quicker Recovery than traditional C02 lasers 7-10 days compared with 4-6 weeks
  • Done with Topical Anesthesia unlike traditional CO2 which requires general Anesthesia
  • Effectively reduces wrinkles, sun induced skin damage and acne scars
  • Tightens skin improving overall tone.

  • My patients specially like tightening of eyelids and jowls
On average, my patients notice, "it takes 5-10 years off "

What is the difference between Fractional C02 Skin Resurfacing and Erbium skin resurfacing?

Erbium laser skin resurfacing is a less invasive treatment. It improves skin texture, superficial pigmentation, fine lines quite well but does not have the same effect at deeper level. Fractional C02 laser penetrates deeper into the dermis than Erbium, therefore leading to more dramatic results for deep wrinkles and scars.

Who is a candidate for CO2 Fractional Resurfacing?

If you have fine lines, some moderately deep lines
If you have rough skin texture, superficial sunspots, poor skin tone
If you have acne or other scars

I have seen a dramatic difference in my patients undergoing this procedure by combining complimentary treatments specific to patients' needs, for example:
I usually combine CO2 skin resurfacing with Botox and injectables for a "non-surgical" facelift effect
For Acne scars, I combine CO2 fractional resurfacing with a series of microdermabrasion for enhanced improvement in texture
For my patients with broken blood vessels, I use vein laser after skin is recovered for a flawless complexion

Liquid Facelift: Dallas Fort Worth Anti Aging Skin Treatment Options In

Non surgical facelift is one of the most requested service in my practice.Aging face begins to lose collagen and elastin leading to sagging eyelids, jowls and nasolabial folds. In the recent years, Physicians have utilized a combination of BOTOX®. and Faical fillers to reduce wrinkles and add volume to face. One interesting observation with this approach to treatment is the effect of "facial lifting".

What is a Liquid Facelift?

Over the years I have used BOTOX® to successfully achieve eyebrow lift in many patients. I have also used a combination of FDA approved dermal fillers mostly to lift and contour the face while reducing deep lines as a secondary benefit.
This approach has acquired the name "Liquid Face Lift", a non-surgical approach to face lift .
Broadly speaking, I divide the face into upper and lower half at lower margin of orbits.I use BOTOX® mostly for lifting and expressive wrinkle reduction of upper half and dermal fillers for voulme restoration, contouring and wrinkle reduction of lower half of face.Additionally, I use dermal filers cautiously for reduction of tear troughs and enhancement of cheek bones.

What can a Liquid Facelift do for you?

Generally a Liquid Face Lift can be used to achieve :
1. Reduction of deep lines, Naso-labial folds, hollows under eyes.
2. A subtle lift of facial profile due to volume restoration and reduction of deep folds
3. Enhancement of facial contours
4. A well rested, natural, youthful appearance

What types of Dermal fillers are used for Liquid Facelift?

I use one or more of the following dermal fillers depending on the area being treated and the effect desired. Juvederm, Juvederm Ultra, Sculptra, Restylane
I have tried a few other fillers but prefer one of the above due to their safety profile and cosmetic outcome. The Dermal fillers I use in my practice are FDA-approved for facial treatment. The approved products may be used "off label" by trained physicians for cosmetic applications not indicated in the manufacturer label. This is because the manufacturers rarely seek FDA approval for multiple facial areas.

BOTOX® is a Muscle relaxant; How does BOTOX® give a "Lift"?

BOTOX® reduces expressive wrinkles by relaxing muscles and causes dropping of eyelids when overdone, which is one of the most common complaint by BOTOX users. Our face has two sets of muscle groups that work in opposition, the "Levators" (muscles that elevate) and the Depressors (muscles that depress). These two groups contract against each other in a constant tug of war.
BOTOX® can be placed strategically to achieve elevation of face by weakening the Depressors thereby letting the Elevators take charge to achieve the lift!

What do you need to know before scheduling your Liquid facelift?

When considering Liquid Facelift you need to take into account the following:
1. Physician's experience with the products
2. The choice of products
3. The goals and expectations from the procedure
Each person is unique and no Two individuals have the same facial features . There is NO "cookbook" method of the injection technique. The Physician has to take into account individual differences and develop injection technique unique to each patient for optimum outcome.

What results to Expect from Liquid facelift?

Liquid Face Lift techniques are evolving and in the right hands the results are outstanding! The best results are subtle, natural and gradual. Most patients notice a "refreshed, youthful" look without a dramatic overnight difference. Individual results vary from patient to patient.
Unfortunately I continue to see patients who have had too much filler or too much Botox and are looking to reverse the effects. In this situation, the best thing to do is wait! None of these injectables are permanent and will eventually disappear. Reversal of BOTOX® is not possible. There is an enzyme, Hyaluronidase that some use to dissolve dermal fillers. I do not use it because it can break down the dermal collagen matrix in addition to the dermal filler, which is worse than where you started!

Ways to Reduce Wrinkles: Anti Aging Skin Treatments in Fort Worth Texas

23 Ways to Reduce Wrinkles read the full story here
Worried that your skin looks older than you feel? Here are 23 ways to reduce wrinkles - starting now!

This Web MD article outlines some of the most common methods used to combat the effects of aging. I incorporate most of these in my practice for may patients.
I like to divide the anti aging measures into the following categories:

  • LIFESTYLE CHANGES

  • TOPICAL SKIN CARE

  • MEDICAL TREATMENTS
LIFESTYLE CHANGES 1.Avoid sun exposure. Photo-aging is the number 1 cause of aging skin. Avoid direct sun exposure between 10 am till 4 pm .Wear sunscreen minimum SPF 15 EVERY DAY. Adequate intake of anti-oxidants like Vitamin C ensures some protection against sun induced damage.

2. Do not smoke. Cigarete Smoking causes reduces blood flow to skin and breaks down collagen and elastin resulting in rough texture, poor color, wrinkles and poor healing.

3. Get the right amount of restful sleep. This ensures adequate production of Human Growth Hormone which keeps the skin thick and supple. Chronic sleep dprivation leads to excess cortisol, a stress hormone that reduces skin blood flow, breaks down skin cells.

4. Sleep on your back. The American Academy of Dermatology (AAD) cautions that sleeping in certain positions night after night leads to "sleep lines -- wrinkles that become etched into the surface of the skin and don't disappear once you're up. Sleeping on your side increases wrinkles on cheeks and chin, while sleeping face-down gives you a furrowed brow. To reduce wrinkle formation, the AAD says, sleep on your back.

5. Don't squint. Any repetitive facial movement, like squinting, frowning, leads to wrinkle formation. Get reading glasses, wear sun glasses. Take break from staring at computer screen and practice relaxation of eye muscles.
6. Eat more fish. particularly salmon. Fish is rich in protein and omega-3 fatty acids which help skin retain moisture.

6. Eat more fruits and vegetables The antioxidant compounds in fruits and vegetables fight free radicals which combats photoaging.

7. Take Vitamin and anti oxidant Supplements: Vitamin D 3, B vitamins, Vitamin C, Vitamin E, Co-Q-10 all have a role in slowing down all aspects of aging including skin aging. Check with your Physician before starting any new supplement.

TOPICAL SKIN CARE

1.Use the right facial cleanser. Pick a cleanser most suited to your skin type, dry sensitive or oily acne prone.Tap water strips skin of its natural moisture barrier.

2.Pick the right skin care ingredients.
Alpha-hydroxy acids (AHAs) These natural fruit acids lift away the top layer of dead skin cells, reducing the appearance of fine lines and wrinkles, particularly around the eyes. New evidence shows that in higher concentrations, AHAs may help stimulate collagen production.

Idebenone, Thiotane and other anti-oxidants. These compounds are related to coenzyme Q10 (CoQ10)is a super-powerful anti oxidant. The fight free radicals and reduce photodamage

Growth factors. When applied topically, may reduce sun damage and decrease lines and wrinkles by inducing collagen production.

Pentapeptides. The results of a study supported by the National Institutes of Health suggested pentapeptides can increase collagen production in sun-damaged skin.

MEDICAL TREATMENTS

1. Botox. The number one requested cosmetic treatment at Docotor's office. Botox injections consist of injecting purified Botulinum Toxin into the expressive muscles under the skin, leading to muscle relaxation and wrinkle release. Very effective, very safe when administered by a trained Physician using legally obtained Botox from Allergan in US. The result is apparent in 3-5 days.

2. Wrinkle fillers. there are many different types of wrinkle fillers available, I have tried many types but prefer to use Juvederm or Restylane in my practice. Both are hyaluronic acid fillers, have fewer side effects (allergic reactions, natural looking results etc.)

3. Intense Pulsed light treatments: ( photorejuvenation). This type of treatment uses an Intense Pulsed light source to target sun induced skin lesions in the superficial skin layer. The treatment is non invasive with minimum to no downtime, minimal discomfort and effectively treats photodamage such as brown sunspots, superficial blood vessels and Rosacea. Photodynamic therapy (PDT) is done with use of photosensitizing agents like ALA to treat deeper skin lesions such as Actinic Keratosis. PDT is also effective in management of Cystic acne and Rosacea.

4. Laser Skin resurfacing. Many different types of lasers are available for skin resurfacing . I use CO2 fractional laser (DOT system) for my patients. A laser removes the top layer of skin, causing a noticeable skin "wounding." This stimulates skin's natural collagen-production system into high gear, resulting in smoother, more wrinkle-free skin. This type of treatment can smooth pitted scarring, textural irregularities and superficial brown discoloration (not melsama).

5. Chemical peels. A chemical acid (instead of laser) is used to "burn" away the top layer of skin, creating damage that causes the body to respond by making more collagen. You end up with younger-looking, smoother skin. It is harder to control penetration of chemicals for deeper peels and can lead to scarring and pigment irregularities in experiencd hands. I use superficial medicated anti inflammatory peels mostly to treat acne and acne scars. occasionaly I may use a custom peels to treat hyperpigmentation in resistant cases.

6. Micro- Dermabrasion. can be achieved with a vacuum suction device used in conjunction with mild chemical crystal or a Diamond tip. Micro dermabrasion helps remove the top layer of skin cells and bring new, more evenly textured skin to the surface. Repeated Micro-dermabrasion at regular intervals increases skin circulation and may induce some collagen synthesis. In the process, fine lines and wrinkles seem to decrease. I use Medical micro dermabrasion in conjunction with comadone extraction and topical Retin A for treatment of moderate papulopustular acne and scars.
